There are the forms related to different functions in the Lake. Please print and use as needed.

Fee Structure	This is the current year’s fee structure that was included with your dues bill.
Membership Agreement	New Members and anyone who has not filled one out in several years is encouraged to do so. Please return to the Lake Wallkill Office address on the left.
Member Survey/Badges	All Members must wear badges and have car stickers. Please fill out this form and return to the Office along with payment made payable to Lake Wallkill Community, Inc.
Permission Slips	If you have children participating in any activity in the Lake, please fill out this form and return to the Chairperson running the event OR you can drop it in the mail slot in the Lake Office.
Covid Addendum Form	COVID ADDENDUM TO THE LAKE WALLKILL CLUBHOUSE RENTAL AGREEMENT – MUST RETURN WITH APPLICATION OF CLUBHOUSE RENTAL TO OFFICE MANAGER.
Clubhouse Rental Agreement	Clubhouse Rental Agreement by a Member in Good Standing (no Alcohol). Please contact Peggy Merck for any questions.
Clubhouse Rental with Alcohol Agreement	Clubhouse Rental Agreement and Alcohol Addendum by a Member in Good Standing. Please contact Peggy Merck for any questions.
Alcohol Consumption Application Form	This application is for Lake Events requesting alcohol. Please fill out and return to office manager.
Guest Form	If you have having guests in for Regatta or another reason, please print the Guest Form and return to the Office as soon as possible.
Deposit Voucher	Use this form to make a deposit for the event you are managing and return to the Office.
Check Request	Use this form to request a reimbursement for monies for the event you are managing and return to Office.
Lake Front Construction Permit	Application for any member that is installing or replacing Construction of a dock, raft or other structure, Construction/Excavation involving the lake shoreline, New or Repair work.
Fence Resolution	Fence Application for any member that is installing or replacing a fence on their property.
